i have been on form submission debuging for 24 hours straight, yet the form isnt submitting

thanks for prompt reply.
https://ippison.vercel.app at the moment of submission, the submit button shows an error on the console saying unable to insert to the database but when I checked database, form data input were saved in the database despite the error messages but documents uploaded on the step 4 of the form isn’t saving in it table. It takes V0 48 hours to debug the issues till now still counting.

Both the form design and creation of database were done by V0 yet his unable to debug it.
